When you receive your Application for Registration, all of the information on the dam and sire wil be printed on the Application. All you need to provide is the information on each calf that is born in the herd. Instructions are below.

- Check the "ET" box if the calf was produced by Embryo Transfer.
List the genetic dam (donor cow) in the second column. On the back of the
form give the breed of the birth mother (recipient cow).

- Give the appropriate disposal code for the dam if her status in the herd
has changed since you subitted the Inventory. Click

- Check the AI box if the calf is the result of artificial insemination.

- List the calf's tattoo. Remember, tattoo must begin with your unique tattoo
and end with the international
year letter.

- For convenience sake, give the calf the same tag number as tattoo.

- List the date of birth of the calf.

- Sex is either "C" for cow or "B" for bull.

- Number Born is either 1 = single birth, 2 = twins or 3 = triplets - note
if you wish to register both twins, use a separate application form for the
second twin.

- CE is calving ease. Click

10 - Give the Birth Weight of the calf. Birth weight should be taken within
24 hours of birth, if possible.
11 - List
the correct color code for the calf. Click

- "Fate" describes what happened to the calf. Fate codes are here.

- Check the "Reg. Now" box if you want the calf registered. ($10
fee per calf) Check "Hold Certificate" if you do not want the calf
registered yet.

- You must name all calves that you want registered. Unregistered calves will
be given a "UMG" number by the computer.
